I just wanted to add because I didn't see it mentioned that you might try
forcing the mac to boot from the cd by changing the image it is looking for
and the boot device in the open firmware. On boot simply hold down option
apple o and f. Once you have this up you can set the boot-device and
boot-file with the setenv command. It is fairly simple and painless if you
are concerned with creating havok in the open firmware use the command
printenv and simply copy down what you see so you can set it back later if
you run into more problems.

I run a rev A upgraded to PPC G4 @433 and aftermarket tray loading cdrw. I
had to dink around in OF a few times to get things to work properly.
