[NTLUG:Discuss] OT - maybe - trying to identify a PCI card

I have been to <http://www.fcc.gov/oet/fccid/> but I have not been able
to identify this card.
Details
9 1/2 inches long by 4 3/16 inches (including PCI pins)
The outside face has 3 "RCA" jacks, and the markings "5001" and
"GLOBE,N.J." and "07092"
The card has: 2 Philips SAA 7146 H chips; 2 AMCC PCI Matchmaker S5933QC
chips; 2 Analog Devices ADV601JS chips; 1 Digital 21152-AA DC1040F chip;
2 9741F V5316258HK40 chips; and a few others.
Markings on the card include: "(c) (M) Intelect 1997 Pat. Pend";
"Starling"; "801-2021-00 REV J 2892"; a barcode "00235136" on face, and
on the reverse: "Pc2A 94V-0 4997"; 171-0730-00 REV C" impressed into the
serface.
The chips and the external connectors (RCA jacks) make me think some
kind of Video or TV output card.
Anyone know this device? or have a suggestion as to where to search?
