I had this happen once.  If I remember right, the problem was a bad type
specification.  So, it is really a PS/2 mouse?  Is that what your
XF86Config says it is too?  Is the mouse device correct, and does it
exist?  (mine is /dev/mouse, which is a symlink to /dev/psaux)  I think
that's all the normal things to check...
